I need help with website stuff

So I'm making a website about dental hygiene and how to tips and ticks about oral hygiene care and post operative instructions and so i think it would best to add a disclaimer just in case ya know. And I want it to say:

Disclaimer
This website is an educational resource only and should not be used to manage your health. All decisions regarding your oral health should be made in conjunction with your Dental Profession or a licensed healthcare provider.

and than I would like for there to be and option for the visitors to acknowledge the disclaimer by selecting an " I acknwledge" button or a "Decline" option just to cover my own ass. and i would really appreciate some help on how to do that and if you do know how to do it and its supper confusing i was wondering if i gave you the user and password to my account...maybe you could just do it for me PPPPleeaaaseee.

if its just a disclaimer it can just be put in small font at the footer of each page.. its not like its a tos or any thing..
